CONSENT FORM FOR INCLUSION IN THE POOL OF EXPERTS ON CLIMATE CHANGE ADAPTATION

Purpose of Data Collection

We are collecting your personal data for the purpose of including you in a publicly accessible pool of experts on climate change adaptation within the framework of the project CICADA4CE. The data will be published on the project website http://cicada4ce.uirs.si/ and in other project materials, making it accessible to the public, partners, and stakeholders.

Legal Basis for Processing

The legal basis for the processing and publication of your personal data is your consent in accordance with Article 6(1)(a) of Regulation (EU) 2016/679 (General Data Protection Regulation – GDPR  link on Regulation - 2016/679 - EN - gdpr - EUR-Lex). By submitting this form, you consent to the processing and publication of the personal data specified herein for the purposes described above.

Personal Data Collected

Photograph, name and surname, country of residency, language, organization, job position, thematic expertise, email adress, LinkedIn profile and a short CV.

Data Retention Period

Your data will be stored for the duration of the project CICADA4CE and its mandatory project retention period but not later than 31 December 2031 or until you withdraw your consent, which you may do at any time by emailing bostjan.cotic@uirs.si or barbara.music@uirs.si.

Your Rights

You have the right to:

  • request access to your data,
  • request correction or erasure of your data,
  • restrict the processing of your data,
  • withdraw your consent at any time (this does not affect the lawfulness of processing prior to withdrawal),
  • lodge a complaint with a supervisory authority (e.g., the Information Commissioner of the Republic of Slovenia at gp.ip@ip-rs.si ).

Consent for the Processing and Public Disclosure of Personal Data

Within the framework of the CICADA4CE project, I voluntarily consent to the collection, storage, processing, and public disclosure of my personal data, including my photograph, name and surname, email address, LinkedIn profile, and short biography/CV, to the extent indicated by my selection below.

I understand that these data may be used for project communication, dissemination, promotion, reporting, networking, and stakeholder engagement activities, and may be published through project communication channels, including the project website, publications, reports, newsletters, social media, promotional materials, and events.

I acknowledge that the provision of my personal data is voluntary and that the legal basis for processing is my consent pursuant to Article 6(1)(a) of the General Data Protection Regulation (EU) 2016/679 (GDPR). I understand that only the categories of personal data for which I provide consent will be processed and made publicly available. I may withdraw my consent at any time by contacting the project partner who collected my data, without affecting the lawfulness of processing carried out before the withdrawal of consent.
